What is PT-141?

An FDA-approved melanocortin agonist for hypoactive sexual desire disorder in premenopausal women.

How it works

Activates MC4R in the hypothalamus, increasing sexual desire through central nervous system pathways rather than vascular ones.

What it has been studied for

  • Directly stimulates sexual desire via brain melanocortin receptors
  • Effective for both men and women
  • Works independently of vascular mechanisms — complements PDE5 inhibitors
  • FDA-approved for HSDD in premenopausal women (as Vyleesi)
  • Can improve erectile function alongside libido
  • Effects begin within 1–2 hours and can last 6–12 hours
  • Useful when psychological factors or hormonal imbalances drive low desire

These are effects reported or investigated in research, much of it preclinical. They are not proven outcomes or health claims.

Published research

  • Approved as Vyleesi (1.75 mg auto-injector) based on the RECONNECT phase 3 trials

Half-life

~2–3 hours

Cautions

Causes transient blood pressure increase; nausea in ~40% of trial participants.

Side effects & fixes

Common side effects

  • —Nausea — the most common effect, usually in the first doses
  • —Facial flushing
  • —Spontaneous erections (Melanotan II)
  • —Darker freckles and moles, and skin tanning
  • —Reduced appetite and frequent yawning
  • —Temporary rise in blood pressure (PT-141)

Histamine reactions

Common flushing and warmth across the face and neck shortly after dosing.

Common fixes

  • Start with a very small dose and increase gradually; nausea fades as you adapt.
  • Dose a few hours after a light meal, and keep ginger or bland snacks on hand.
  • Avoid if you have uncontrolled high blood pressure or heart disease.
  • Check your moles regularly and have any that change looked at by a dermatologist.
  • For mild itching, redness or flushing, a non-drowsy over-the-counter antihistamine (such as cetirizine or loratadine) is commonly used — check with a pharmacist first, especially if you take other medicines.

Stop and get help

  • A painful erection lasting more than 4 hours.
  • Any mole that changes shape, colour or size.
  • Stop and get emergency help for swelling of the face, lips, tongue or throat, trouble breathing, widespread hives, chest pain or fainting.

Medication concerns

From FDA prescribing information
  • Naltrexone taken by mouth (for alcohol or opioid use)

Bremelanotide can sharply lower naltrexone levels. The label says to avoid using them together.

  • Antibiotics and pain relievers taken by mouth

It can slow stomach emptying, reducing or delaying absorption; avoid it with drugs that need a steady level, such as antibiotics.

  • Blood pressure medicines, heart disease

It temporarily raises blood pressure and lowers heart rate after each dose.

  • Erectile dysfunction pills (sildenafil, tadalafil)

Not studied together; the combination may raise the risk of prolonged erection or blood pressure swings.

  • Everything else you take, including supplements

Bring a full list of medicines and supplements to your pharmacist or clinician before starting.
